NYBDC Forms
The Loan
Application Checklist is a tool for borrowers to
accumulate the necessary information NYBDC will need to evaluate
your business. Please include with your application any brochures
that may explain your business. This represents the majority of
the information required by our formal application. Your NYBDC
loan officer can provide you with their thoughts about your project
with this information prior to making a formal application for
a loan.
The NYBDC Loan Application is
a longer version of the above checklist.
SBA Loan Forms
Personal Financial Statement
Form (SBA 413) - This
form needs to be signed by the applicant and spouse (if applicable).
Anyone who owns 20% or more of the business or operating company
needs to complete.
Statement of Personal History Form (SBA
912) - All owners of the business and/or operating company needs
to complete this form. If you are not a U.S. Citizen, you will
need to attach a copy of your Registration Card.
Request for
Transcript of Tax Return (4506-T) - Your NYBDC loan officer
will complete the upper portion of this form for each company you
own. The form must be signed by the President of the company.
Counseling
Information Form (SBA 641) - Your NYBDC loan officer will assist
you in completing the top portion of this form.
Debt Schedule -
This form shows a list of your business loans; mortgages, equipment
loans, vehicle loans, lines of credit, etc.
Management
Profile - Resume -
This is a simple resume format. It should be completed by each
owner of the business. If you have your own resume format that
will be acceptable.
U.S. SBA Application
for Business Loan (SBA 4) - This form is to be used for a loan
that will have an SBA Guarantee. Your NYBDC loan officer will assist
you in completing this form.
Certification Regarding
Debarrment (SBA 1624) - This form is to be used
for a loan that will have an SBA Guarantee, it is not needed for
a 504 Loan.
Application for Section 504 Loan (SBA
1244) - This form is to be used for a 504 Loan only. The borrower
only completes Section C. Section C begins on page 8 and requires
signatures on page 11. Your NYBDC loan officer will assist you
in completing this form.
Seismic Letter -
This form is for a 504 Loan, only when there is new construction
of a building or the expansion of an existing building. The form
would be signed by the borrower's building contractor or architect
and confirms that the plans meet the federal seismic standards
in the building code. This letter would be signed after the building
is completed.
